We at SureServ value your privacy as much as we value your access to affordable healthcare products and services. This Privacy Notice describes the information SureServ and our affiliates (collectively, “SureServ,” “we,” and “us”) collect when a person or business (“you,” “your”) applies or signs up for Account in order to access and use the SureServ Application (“App”).
Your Consent.
By applying or signing up for an Account via any and all devices, you accept the terms of this Privacy Notice and give your express consent to our collection, use, disclosure, and retention of your personal or sensitive information as described herein, and your explicit authorization to contact you, and for all other purposes permitted under applicable personal information privacy statutes, credit bureau reporting rules, anti-spam legislation, and consumer protection laws.
Amendment.
We may amend this Privacy Notice from time to time by posting a revised version and updating the “Last Updated” date above. The revised version will be effective at the time we post it. We will provide you with reasonable prior notice of substantial changes in how we use your information, including via the email address connected to your Account. If you do not agree to any of the terms, you may terminate your Account at any time. We encourage you to review this Privacy Notice whenever you access or use in any manner any SureServ application, product, service, feature, technology, content, or website (collectively, the “App”) to stay informed about our information practices and the choices available to you and to contact us if you have any questions about these changes. Your continued use of our App constitutes your acceptance of any amendment of this Privacy Notice.
Why do we collect your personal or sensitive information?
We collect your personal information to:

Perform functions such as but not limited to processing, reviewing, and declining or approving, as the case may be, your application in connection with the access and use of the App, or when necessary, terminating your Account;

Process your availments and transactions, including details of payments;

Conduct further research for the purpose of reviewing, developing, and improving our App and associated products and services and other functions required or permitted by law;

Perform analysis through profiling, user behavioral modeling and analytics to better understand the needs, preferences, and market trends in order to improve our App and associated products and services;
Our App can be used in a broad range of industries in connection with their products, services, activities and administrative functions, whether they are involved in government, retail, health, technology or anything else. This means that a broad range of information (including information about you) may be uploaded to or sent through our App. You have the choice, at all times, not to provide your personal or sensitive information. However, without information about you, the App may stop functioning or we may not be able to provide you with the products and services related to the App or the support you request.
Our App (and website) are not targeted or directed at persons under the age of 18. As the Account Holder, you expressly agree that you are responsible for any and all information you give us about persons under the age of 18 in connection with your Account.
What types of personal or sensitive information do we collect?
When you apply, or sign up for an Account, we may collect personal or sensitive information as part of the set-up and identity or account verification process, such as but not limited to:

Identification, contact, and demographic information, such as your name, age, email address, mailing address, phone number, photograph, birthdate, passport, driver’s license, Social Insurance or other government-issued identification number (including a photograph of such identification card);

Identification, contact, and demographic information about the minor persons associated with your Account, such as name, age, mailing address, photograph, birthdate, passport, health or medical history, status, records, and risks (including a photograph of such identification card);

Professional information (for medical professionals);

Financial information, credit history, payment history, including payment gateway accounts or bank account and payment card numbers;

Tax information, including withholding allowances and tax filing status; and

Other historical, contact, and demographic information.
We also collect information you upload to or send through our App, including but not limited to:

Transaction information and other information about products and services you may buy (including inventory, pricing and other data), including how you use our App to make, accept, request, record, or pay availments, when and where the transactions occur, the names of the transacting parties, a description of the transactions, the payment amounts, billing and shipping information, and the devices and payment methods used to complete the transactions.

Information you may provide about you and any person in connection with you use of the App (including appointment, health history, status and risks, and contact data).

Non-personal information, use, and device information such as those provided by your device, which may include how you use our App such as your access time, language, hardware model, operating system and version, unique device identifier, mobile network information, and information about the device’s interaction with our App, geolocation, IP address, other software running on the device for anti-fraud and malware-prevention purposes (but we will not collect any content from such software), and other identifiers..

User feedback, including information you share with SureServ such as your response to surveys, questionnaires, interviews, and your comments and opinions regarding the App, and associated products, and services, and voice recordings, if any, when you otherwise communicate with us; and,

Where do we get your personal or sensitive information?

Information Collected When You Create or Use Your Account.
As we set out in detail above, we collect information when you apply, create, use, or otherwise access your Account.
We use various technologies to collect information when you use our App, including sending cookies to your mobile device or tablet/gadget and using web beacons. Cookies are small data files that are stored on your hard drive or in device memory when you visit a website or view a message. Among other things, cookies ensure the integrity of our registration process, retain your preferences and account settings, and help evaluate and compile aggregated statistics about user activity. We also may collect information using web beacons. Web beacons are electronic images that may be used in our Services or emails. We may use web beacons to deliver cookies, track the number of visits to our website or your usage frequency of the App, understand usage and campaign effectiveness, and determine whether an email has been opened and acted upon. Information We Collect from Other Sources.
We also may collect information about you from third parties who are authorized to give information about you under the terms and privacy policies of their respective services. Such third parties that may share information with us include third-party verification services, credit bureaus, mailing list providers, and publicly available sources. In some circumstances, where lawful, this information may include your Social Insurance or other government-issued identification number. By applying or signing up for an Account, you authorize and consent to our obtaining from, and disclosing to, third parties, from time to time, any information about you in connection with the processing of any credit scoring, credit investigation, identity or account verification, fraud detection, or collection procedure, or as may otherwise be required by applicable law. This includes, without limitation, the receipt and exchange of account or credit-related information with any credit scoring, credit reporting agency or credit bureau, where lawful, and any person or corporation with whom you have had, currently have, or may have a financial relationship, including without limitation past, present, and future places of employment and personal reporting agencies.
How do we use your personal or sensitive information?

When and to whom do we share your personal or sensitive information?
We may, and we may use third-party service providers to, process and store your information in other countries. While we take measures to ensure protection of your information, governments, courts, law enforcement or regulatory agencies in other countries may be able to obtain disclosure of information through the laws of these countries.
We may share information about you, such as but not limited to as follows:

With our group companies and other affiliates, for the purposes outlined above, in the EULA, in the Terms of Service, and other related agreements;

We also may share aggregated information with third parties that does not specifically identify you or any individual user of our Services.In all cases, we only share your information to third parties that is necessary for them to perform their functions and to the extent permitted for by law.
SPECIAL PROVISION FOR LENDING COMPANIES
Pursuant to Republic Act (R.A.) No. 9510 and its Implementing Rules and Regulations (IRR), creating the Credit Information Corporation (CIC), we are mandated to submit your basic credit data (as defined in R.A. No. 9510 and its IRR), as well as any regular updates or corrections thereof, to the CIC for consolidation and disclosure as may be authorized by the CIC. Consequently, your basic credit data may thus be shared with other lenders authorized by the CIC, and other credit reporting agencies duly accredited by the CIC, for the purpose of establishing your creditworthiness.
Your Choices.

Personal Information. You may access, change, or correct information about you by making a request to us using the contact details below, in which case we may need to verify your identity before granting access or otherwise changing or correcting your information.

Deactivating Your Account. Subject to the terms of the EULA and Terms of Service, you can deactivate and terminated your Account by emailing us using the contact details provided below. We generally retain information about you only as long as reasonably necessary to provide you the App. However, even after you deactivate your Account, we may retain archived copies of information about you and any transactions using the App, in which you may have participated for a period of time that is consistent with applicable law, or as we believe is reasonably necessary to comply with applicable law, regulation, legal process, or governmental request, to prevent fraud, to collect fees owed, to resolve disputes, to address problems with our App, to assist with investigations, to enforce our EULA, Terms of Service, or other applicable agreements or policies, or to take any other actions consistent with applicable law.

Location Information. In order to provide certain mobile applications, we may require location information, so if you do not consent to collection of this information, then you cannot use our corresponding App. You can stop our collection of location information at any time by changing the preferences on your mobile device. If you do so, our mobile application will no longer function. You also may stop our collection of location information via mobile application by following the standard uninstall process to remove all SureServ mobile applications from your device.
Cookies.
We try to make your experience with SureServ simple and meaningful. When you access or use the App, our system may send a cookie to your computer, mobile device, tablet, or gadget (as the case may be). Certain cookies we use last only for the duration of your session and expire when you close or exit your web browser or the App. Other cookies are used to remember you when you return to use the Services and, as such, will last longer.
We may use cookies to:

Remember that you have visited us or used the App before. This means we can identify the number of unique visitors we receive, which allows us to make sure we have enough capacity to accommodate all of our users.

Customize elements of the promotional layout and/or content of our website or in App.

Collect anonymous statistical information about how you use the App (including the length of your session) and the location from which you access the App, so that we can improve the App and learn which elements and functions of the App are most popular with our users.
Some of the cookies used in the App are set by us, and others are set by third parties who deliver services on our behalf.
Most web and mobile device browsers are set to automatically accept cookies by default. However, you can change your browser settings to prevent automatic acceptance of cookies, or to notify you each time a cookie is set.
Please note, that by blocking or deleting cookies used in the Services, you may not be able to take full advantage of the App.
Promotional Communications
You may opt out of receiving promotional messages from SureServ by following the instructions in those messages, or by changing your notification settings by logging into your Account. If you decide to opt out, we may still send you non-promotional communications, such as digital receipts and messages about your account or our ongoing business relations.
How do we protect your personal or sensitive information?
We take reasonable measures, including administrative, technical, and physical safeguards, to protect your information from loss, theft, misuse, and unauthorized access, disclosure, alteration, and destruction. We hold information about you on our secure cloud-based servers from service providers such as, but not limited to, reputable third party providers. We restrict access to personal information to those SureServ employees, contractors, and agents who need to know that information in order to transmit, store, or process it, who are subject to contractual confidentiality obligations consistent with this Privacy Policy, and who may be disciplined or terminated if they fail to meet these obligations. Our third-party service providers store and transmit personal information in compliance with this Privacy Notice and other appropriate confidentiality and security measures.
Nevertheless, SureServ cannot guarantee that unauthorized third parties will never be able to defeat our security measures or use your personal information for improper purposes. In the event that any information in our possession or under our control is compromised as a result of a security breach, we will take reasonable steps to investigate the situation and, where appropriate, notify those individuals whose information may have been compromised and take other steps in accordance with applicable laws or regulations.
Your rights to your personal information
In accordance with the Data Privacy Act, we uphold your right in relation to your personal information. You have the right to be informed on the processing of your personal information and you also have the right to object to our collection or processing of your personal information. Further, you have the right to access, to correct or modify, or erase your personal information from our systems and processes and you also have the right to withdraw your personal information from any of our mailing list whether for service, announcements or sales and marketing purposes. However, choosing not to disclose or deciding to remove your personal information from our App and systems may not only limit our ability the functioning of our App but result in the App no longer functioning.
